Systemic mastocytosis with associated clonal hematological nonmast cell lineage disease

A 39-year-old male presented with compressive myelopathy due to epidural mass. Work up for multiple myeloma/plasmacytoma was done before decompression surgery. Peripheral smear showed large cells probably the mast cells and no blast cells. Bone marrow aspiration and biopsy showed a collection of atypical mast cells which are CD117 positive [Figure 1] and [Figure 2]. No blast cells were seen in bone marrow study. Later, the excision biopsy of epidural mass showed myeloid sarcoma, possibly myelomonocytic type with mast cell proliferation (myeloperoxidase positive, CD 117 positive in large cells, CD 68 positive, CD 34 negative, CD 56 negative, CD 33 inconclusive) [Figure 3]. Diagnosis of "Systemic mastocytosis with associated clonal hematological non mast cell lineage disease" was made with WHO criteria for the systemic mastocytosis. After decompression surgery, he was started on chemotherapy for acute myeloid leukemia but succumbed to his illness. | Figure 1: Collection of mast cells in bone marrow trephine biopsy H and E ×400
